Remote Learning Program Enrollment Application

This application must be filled out for each student seeking enrollment into The Academy's Remote Learning Program for the 2022-2023 school year. There is a $45 application fee per student that is required with the submission of this form.

All admissions decisions are made after carefully examining the applicant’s previous academic records and considering the well-being of both the student and The Academy for G.O.D. Remote Learning Program as a whole.

By completing this form you are certifying that all information provided is true and correct according to the best of your knowledge, and that you understand the scope of the Academy for G.O.D. Remote Learning program.
